THE FOLLOWING MUST BE INCLUDED FOR CONSIDERATION AS A STUDENT IN THE CNA PROGRAM

1. Placement test scores of English 90 or ESL 150 or completion of AA degree or higher

2. Fingerprinting - Completed live scan process

3. Program physical form completed and signed by health care practitioner.

4. TB clearance current for year of participation in program, (either a negative PPD or clear chest xray).

5. Immunizations or proof of immunity to the following: measles, mumps, rubella, varicella, hepatitis B(need to have begun series if not immune).

6. Current CPR certification.

7. Signed that read and understood "Disqualifying Penal Code Sections"
